<h2>Goal</h2>
<p>This concept aims to support research to identify, optimize, and test solutions to enhance the effectiveness, quality, and availability of U.S. suicide prevention services.</p>
<h2>Rationale</h2>
<p>Research has yielded evidence-based suicide prevention strategies and tools for screening, risk stratification, and interventions and services to reduce suicide risk. Yet, the lack of scalable approaches for training, supervising, and monitoring the quality of delivery by clinicians, peer specialists and other paraprofessional providers leaves many providers feeling ill-equipped to serve some of their most vulnerable service users, and limits overall access to effective, evidence-based suicide prevention. The significant behavioral health service provider shortage affecting more than a third of the U.S. population compounds the problem. This further underscores the need for a more robust, complementary, and well-trained provider base, including sustainable strategies for integrating and expanding the number of effective peer support specialists and paraprofessionals into clinical and community practice settings.</p>
<p>The proposed concept would foster research that tests solutions to improve the effectiveness, quality, and availability of suicide prevention services. One area of emphasis would be to support studies focused on optimizing and testing provider training strategies and quality monitoring tools to promote competence and sustained implementation fidelity in delivering evidence-based suicide prevention interventions and services. Examples could include studies testing research-supported provider-level training and supervision strategies, validating scalable measures and tools for monitoring delivery and outcomes of suicide prevention services, and/or examining novel technologies to support training and real-time monitoring of intervention delivery and quality. Additionally, the concept encourages research developing and testing strategies to augment clinical services through scalable and sustainable peer support interventions that complement and extend services for suicide prevention in clinical and community settings. Studies are encouraged that examine task sharing models that propose and test clear roles for Peer Support Specialists (PSP) and incorporate scalable and sustainable approaches to training and supervision, safety monitoring, and credentialing to complement and extend the behavioral health workforce. PSP strategies should be based upon relevant conceptual frameworks that inform provider roles, duties, and factors presumed to underlie the intervention effects. Models could be tested across clinical and community practice settings, including the crisis care continuum and within stepped care protocols. Models should be informed by those involved in intervention models of behavioral health services, including service users and others with lived experience, providers, and administrators who make decisions about adoption, credentialing, and reimbursement. Studies could also leverage data from health records or other administrative data sources to evaluate innovative approaches to deploying peer support services, including approaches that involve telehealth and innovative reimbursement policies.</p>

Research is also needed to optimize existing technology and understand factors related to engagement and sustainability of digital health platforms.</p>
<h2>Rationale</h2>
<p>Digital health technology offers unprecedented opportunities to help consumers, clinicians, and researchers measure, manage, and improve health. These tools also have the potential to improve our understanding of mental illness across the lifespan, track the course of illnesses and recovery, and provide and enhance mental health care. Most importantly, digital health interventions offer the potential to bridge the treatment gap and provide evidence-based interventions to the many individuals who currently are unable to access treatment.</p>
<p>Over the last decade, NIMH has supported the development and testing of digital health technology, with a focus on establishing the efficacy of digital health assessments and interventions. This research has demonstrated that digital health technology can be used to <a href="https://www.jmir.org/2015/7/e175/" rel="external noreferrer noopener" target="_blank">passively monitor clinical states</a> <a href="http://www.nimh.nih.gov/site-info/policies#part_2717" title="Exit Disclaimer" class="exit-disclaimer" target="_blank" rel="noopener"><i class="fa-solid fa-arrow-up-right-from-square ext-link-icon"/></a> and reduce symptoms. However, with few exceptions, most NIMH-funded research in digital health technology has focused on efficacy with less emphasis on effectiveness and dissemination. In contrast to the federally funded mental health research, the pace of commercial technology development moves rapidly. Over the last five years, commercially available digital health platforms for mental health have gained considerable traction in the marketplace. Current estimates suggest that <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5897664/" rel="external noopener" target="_blank">greater than 25% of commercially available digital health apps <i class="fa-solid fa-arrow-up-right-from-square ext-link-icon"/></a> focus on mental health and a significant cross section of consumers are regularly utilizing digital health technology to access treatment for mental health. As digital health technology for mental health is being increasingly used to provide standalone self-managed interventions and/or to supplement in-person treatment, well-designed research is necessary to evaluate the effectiveness of existing digital health technology.</p>
<p>This initiative would encourage research that promotes partnerships between academic investigators digital health companies. These partnerships will enable research to be conducted with large samples of participants and will leverage existing digital health platform infrastructure. It is expected that these partnerships will enable both nimble studies to rapidly test ideas and conduct exploratory research as well pragmatic trials of digital health interventions. The general scope of research would include developing and testing:</p>
<ul>
<li>Strategies to increase the reach, efficiency, and quality of digital health interventions.</li>
<li>Factors related to engagement, uptake, and sustainability of digital health platforms.</li>
<li>The effectiveness of digital health platforms to optimize the benefit of in-person treatment and bridge therapy sessions and promote between-session skill practice/acquisition.</li>
<li>Technology driven approaches to improve access/engagement and continuity of care during known periods of heightened risk (e.g., handoffs between emergency departments and inpatient psychiatric; transitions between primary care settings and outpatient mental health programs).</li>

<p>Therapeutic interventions are now being applied in ways that may enhance timely and tailored strategies to address suicide risk. This approach requires ready access to end-user data that informs proactive intervention strategies. This webinar will consider 1) which data sources are best equipped to inform intervention strategies, 2) which intervention aspects should involve tailoring strategies vs. simpler approaches that may be just as effective, and 3) how such approaches can be incorporated into providers’ workflows within a health care setting. </p>
<p>This webinar brings together experts in areas that include passive and active data collection methods, the measurement of social contexts related to suicide risk, and the methodological aspects of Just-in-Time Adaptive Interventions (JITAI). The goal is to provide insights into developing and testing JITAIs for suicide prevention and to offer guidance on incorporating these adaptive interventions into healthcare systems. By participating, attendees will gain valuable knowledge on how to advance and implement effective suicide prevention strategies.</p>
